Parent Teacher Conference Student Sharing Form

This form is an awesome tool for you to share with families at conferences! As a teacher and mom, I have discovered that besides the academic info, a lot of what families are interested in is what their child is like at school and who they are friends with, etc. 

This sheet is designed to be filled out (or dictated) by the students and shared at conferences or open house. It will be a great, honest conversation starter between you and parents and also between students and their parents. I've used this form for many years and it's consistently been a source of smiles, connections, and valuable information.

There is a form included and personalized for each grade: preschool, kindergarten, 1st, 2nd, 3rd, 4th, 5th, and 6th. They each have the same basic wording, but the questions are relevant to all ages of students. Check out the preview for a closer look. :)
